Kids Ministry Pre Check-In Form

One of our core values at HVC is coming alongside parents to Build Healthy Families. We are so honored to be a part of your child's life and prioritize the safety and security of our ministry environments!

To help us do this, we operate our kids ministry within a closed environment where only registered children, their guardians, or screened ministry team members are allowed.

You can fill out the form below to pre-register your kids now so their information will be ready for check-in when you join us for a service.

When you arrive, simply walk up to the Kids Ministry Check-in Desk, let the team know that you're new and that you completed this form. They'll help you with any questions and show you around!

I understand that HVC will not change diapers and will not assist children who cannot use the bathroom by themselves. *

I understand that HVC will notify me by SMS message or by pager (nursery) if my child needs assistance. *

I understand that no one is allowed in the children's areas without providing a valid Security Tag (printed with name tags at Check-in). *

I understand that at pick-up, only ONE adult is allowed in the children's areas and that they must show their matching Security Tag to the Safety Team member. (NOTE: This greatly helps us reduce crowding.) *

I understand that, on occasion, photos & videos may be taken for church use including promotional materials and social media. To request that your child not be included, please talk to the check-in desk team. *
